Book an Appointment

Select a convenient consultation time. Prior to your initial appointment, you'll need to complete a New Patient Form. This form will be sent to the email address you used to book in for the appointment

Telehealth Consultation

We'll send you reminders for your appointment via email. Your practitioner will contact you for a telehealth consultation using a private number, ensuring a seamless and efficient experience. Please ensure your phone settings allow private No caller ID calls before your appointment as all calls will appear as No Caller ID calls.

Medication Delivered to Your Door

If your practitioner prescribes any medicinal cannabis medication, you can order and purchase it directly through our dispensing arm saving you time and effort.

Booking form for new patient

Are you a new patient or existing patient? Note that you are considered a new patient if you have seen Dr Kevin in other clinics but it is your first time seeing Dr Kevin via Medicanna Health as we will be required to onboard you into our system. If you are a new patient, please use the Initial Consultation option below.

If you are a returning patient and have seen Dr Kevin via Medicanna Health before, please use the Review Consultation option below.

Disclaimer: Unfortunately, we are unable to assist any patients who are on Methadone or Opiod treatment program or any patient with a history of Drug Dependency or history of Schizophrenia/Psychosis. We are also unable to take on patients who are under the 18 years of age.

We may require a recent Health Summary from your GP or Specialist prior to the commencement of any treatment.

Please note there are certain criteria that need to be satisfied before the commencement of any treatment can be undertaken and prescribed by our doctor. Please note that an appointment with a doctor is not a guarantee of any Medicinal Cannabis prescription.
