Once you have booked a consultation with our doctor, you will receive an email with a patient form. Please ensure you complete and submit the patient consent form prior to your appointment. Our doctor will then have the information to proceed and efficiently consult with your conditions. The doctor will evaluate your conditions and if clinically appropriate a prescription will be supplied. On the day of consultation, our doctor will contact you via the phone number you have provided.
